Practitioner verifies and signs the PT33
A licensed practitioner or partner clinical provider verifies the completed application and digitally signs the PT33.

Digitally signed PT33 is delivered
The patient receives a PT33 PDF digitally signed by the doctor, together with instructions for presentation to licensed dispensaries in Thailand.
What Patients Should Prepare
- A valid passport with readable identification details
- An email address you can access while travelling
- Select your condition from the available options
Passport details are read automatically. The form also asks for your phone number and gender, your chosen prescription start date and your signature.
Important Boundaries
- All completed applications are approved and receive a PT33 PDF digitally signed by the doctor.
- If we are unable to issue your prescription, including because of technical issues, you will receive a full refund.
- The service is not emergency medical care.
- Foreign prescriptions do not replace Thai practitioner review.
- Dispensaries may still perform their own document checks.
- Rules can change, so official guidance should be followed.
