One process for visitors and residents
Our service accepts applications from adults aged 20 or over who have a medical reason for applying and are in Thailand or planning a visit. The published requirements apply nationwide, including Bangkok, Chiang Mai, Phuket, Pattaya and Koh Samui.
Select the condition that applies to you from the options on the form. You do not need to describe symptoms in writing or provide medical records from home. Contact support before payment if none of the listed conditions applies.

The form also asks for your phone number and gender. Choose your arrival date or another prescription start date, then add your signature with your finger or mouse.

Choose when your prescription starts
You can apply before travelling to Thailand. In your application, select your arrival date or another date when you want your prescription period to start. Your prescription is valid for 30 days from that selected start date.
Once your signed PT33 arrives by email, check that the dates match your selection and save the PDF for your dispensary visit.
The service fee is ฿495. All completed applications are approved; the clinic verifies and signs them. Read the national guide for office hours, delivery and the refund commitment.
